An announced, on-site monitoring inspection was initiated on 1/28/2025 and completed on 1/28/2025, as a part of the conditional licensure period. The on-site inspection began at 10:57AM and ended at 1:08PM. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility and measurements.

Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. An outlet in the 1-2 years class was not secured flush with the wall at the bottom of the outlet.
Plan of Correction: Provider will ensure outlet is mounted flush with the wall.
