Inspector Notes

An unannounced, on-site renewal inspection was initiated on 7/9/2025 and completed on 7/9/2025, as a part of the licensure period. The on-site inspection began at 12:48PM and ended at 2:29PM. The inspector reviewed compliance in the areas listed above. There were 7 children present and 6 staff. The inspector reviewed 5 children?s records on site on 7/9/2025 and 3 staff records on 7/9/2025.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #2, was employed for 1 day before a completed national criminal background check was received. Staff #3, was employed for 1 week before a completed national criminal background check was received.
Plan of Correction: Provider will obtain fingerprints prior to first day staff work with children.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who had been attending for two weeks, did not have documentation of an immunization record on file.
Plan of Correction: Provider will correct by 7/11/2025.
Standard 8VAC20-780-450-A
Rest mats used by specific children other than infants shall have linens consisting of a top cover and a bottom cover or a one piece covering which is open on three edges. 1 child did not have a bottom cover on their rest mat in classroom 1.

REPEAT VIOLATION
The following staff records shall be kept for each staff person: the following required components were missing. Staff #1, employed for over a month, did not have documentation of an address of an emergency contact. Staff #2 record, employed for 2 weeks, did not have documentation of a job title and address of an emergency contact. Staff #3, employed for over a month, did not have documentation of a job title.
