During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
This Inspection notice was amended on 06/17/2025. The provider has been provided with a copy of the updated inspection notice.
An initial subsidy health and safety inspection (SHSI) was initiated on 06/16/2025 and completed on 06/16/2025 in conjunction with a monitoring inspection. Subsidy standards that are not covered in voluntary registration standards were reviewed during this SHSI. The inspection started at 1:17pm and concluded at 2:06pm. There were zero children present and one caregiver; the point total was zero. The inspector reviewed zero children?s records and one caregiver record on-site. This inspection included a tour of the facility, document review, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/24/2025.

2The first aid kit did not contain a thermometer and a pair of tweezers at the time of inspection.
A battery-operated radio was not available at time of inspection.
