Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 4/14/25 from 10:40 am to 11:25 am, as a part of the conditional licensure period. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 8. The inspector reviewed 4 children?s records. This inspection included document review, tour of the family day home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination within 30 days after the first day of attendance.
Child # 3 and Child # 4 have both been attending the family day home for over 30 days. The provider did not obtain documentation of their physical exams to this date.
Plan of Correction: Documentation of the physical exam will be requested from the parents.
Standard 8VAC20-800-60-B
Children's records must contain complete information.
The following information was missing from children's records: Child # 1 liability insurance form; Child # 2, Child # 3 and Child # 4's record was missing the liability insurance policy form and the medication authorization policy form; Child # 2 and Child # 3's record was missing the medical insurance policy information.
Plan of Correction: All missing documents will be obtained from the parents.
