Inspector Notes

An unannounced, on-site initial inspection was initiated on January 30, 2025 and completed on January 30, 2025. The on-site inspection began at 10:00am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were no children present and 1 staff. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/7/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. The following safety issues were observed:
1.The indoor play area material shelves had containers that held small plastic buttons, small plastic teddy bears and foam shape pieces that were small enough to be a potential choking hazard.
2.The outdoor play area had multiple wooden framed chalk boards that had spaces in between the corners of each frame that could cause a potential snagging or splintering injury.
3.The restroom had a pack of plastic garbage bags in a cabinet unlocked and accessible to children causing a potential suffocation hazard.
Plan of Correction: The small plastic buttons, foam shapes and teddy bears have been put away and out of reach.

The chalk board has been filled in and sanded down to avoid splintering.

The plastic trash bags have been removed and placed in a storage closet.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. The designated disposal system next to the diaper changing table did not have method open it without using hands.
Plan of Correction: The changing table trash can has been replaced with a foot operated trash can.

The following nonmedical emergency supplies shall be required: One working, battery-operated radio in each building used by children. The center did not have a battery-operated radio.
Plan of Correction: A battery operated radio has been purchased and is located int he office.
