Inspector Notes

An unannounced, on-site renewal inspection was initiated on July 14, 2025 and completed on July 14, 2025. The on-site inspection began at 10:50am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/24/2025. A POC submitted after this date will not appear on the public website.

When food is brought from home the food container shall be clearly dated and labeled in a way that identifies the owner. Multiple lunch boxes and water bottles inspected did not have names and were not dated.
Plan of Correction: All lunchboxes will be checked for first/last name, and date. We will keep tape and a marker near cubbies to insecure any/all food/drink from home is labeled upon arrival.

Documentation of at least two references to character, reputation, and competency are to be checked prior to employment. Staff #1, who was present and working at the center only had one reference verification on file.
Plan of Correction: A second reference for staff #1 was printed from online employee file, documented on a newly implemented checklist, and filed accordingly.
