Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. The record for staff #1 did not have documentation of a completed staff orientation and has been working at the center for at least six months.
Plan of Correction: Response from the center:
The identified staff member?s orientation paperwork was immediately recovered and placed in the staff file. She had in fact completed her required orientation training before her first day of employment, and documentation verifies that. The Director also created a new Employee Application and a documented staff onboarding checklist to be verified of completion prior to scheduling any future staff.

REPEAT VIOLATION
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There were multiple tubes of diaper cream and a bottle of hand sanitizer on the children?s storage shelf. All items had the ?keep out of reach of children? label and were unlocked and accessible to children.

Plan of Correction: Response from the center:
Immediately upon notification, all diaper creams and hand sanitizer were removed from accessible shelving and placed in a locked cabinet and secured areas that are inaccessible to children. Staff were informed immediately of the requirement that all items labeled ?keep out of reach of children? must be always stored in locked locations. The Director will perform daily inspections of the classroom to ensure hazardous materials remain properly secured.

The missing information has been completed and confirmed by the families and Director, and ALL child records have been updated to include complete addresses and work contact information for parents and emergency contacts, not just on paper but in their online student file as well. Now, all files are easily accessible and gathered in one safe place to be audited monthly to ensure all files are correct and up to date. Also, a student on-boarding file review checklist has been implemented to ensure all required information is documented and saved efficiently.
The missing information has been completed and confirmed by the families and Director, and ALL child records have been updated to include complete addresses and work contact information for parents and emergency contacts, not just on paper but in their online student file as well.
