An announced, on-site initial inspection was initiated and completed on January 29, 2025. The on-site inspection began at 1:43 pm and ended at 3:17 pm. The inspector reviewed compliance in the areas listed above. There were no children present at the facility. The inspector reviewed all required attachments for the initial application process. This inspection include document review; tour of the facility, interviews, and observations.
