During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Inspector Notes

An unannounced inspection was initiated on May 30, 2025 and completed on July 1, 2025 in response to a complaint received by the licensing office on May 27, 2025 and May 29, 2025 relating to allegations of programs, staffing and supervision, special services. The inspector observed 18 children in care and 2 staff supervising. This inspection included interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid for programs, and special services. However, the allegations for staffing and supervision was deemed valid and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 24, 2025. A POC submitted after this date will not appear on the public website.

When children are in ongoing mixed age groups, the staff-to-children ratio and
group size applicable to the youngest child in the group shall apply to the entire group.

The staff to children ratio in the Four/Five room (the youngest children age 2 years) there were 18 children present with 2 staff. The required ratio for children age 2 years is one per eight children.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

At approximately 8:52 a.m. on 5/30/2025, there were 18 children present in the Four/Five room, but only 12 were marked present in the attendance record app.
