During the inspection, the inspector reviewed the areas listed above.

Inspector Notes

An announced initial inspection was initiated and completed on 12/10/24 as a result of a planned change in ownership. The on-site inspection began at 10 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. Two staff records were reviewed on site. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside shall be maintained in a safe condition.

1. On the chain link fence and gate between the two playgrounds were protruding screws and nuts that presented an impalement hazard.
2. The 3 pieces of play equipment on the Infant/Toddler/Two's playground were not anchored and would easily moved around with usage or if pushed.
Plan of Correction: We will reach out to the seller to make the necessary repairs before the sale is complete.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall be in compliance with the standards, to include at least six inches of resilient surfacing underneath and surrounding the equipment as well as proper use zones under equipment with moving parts or climbing apparatus to create a fall zone of at least 6 feet free of hazardous obstacles.
1. On the Infant/Toddler/Twos and the Pre-K/School age playgrounds the mulch had degraded to soil therefore there was no acceptable resilient surfacing ( mulch) under the play equipment for each playground.

2. On the Infant/Toddler/Twos playground the trikes attached to the metal track as well as the composite play structure were within 6 feet of the wooden border.
