Inspector Notes

An unannounced inspection was initiated on 5/22/25 and completed on 6/16/25 in response to a complaint received by the licensing office on relating to allegations of staffing, supervision, physical plant, staff qualifications and training.The inspector reviewed 7 staff records on site on 5/22/25 and electronically on 5/23/25. This inspection included document review, tour of the facility, interviews, observations and measurements.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/24/25. A POC submitted after this date will not appear on the public website.

Where playground equipment with moving parts or climbing apparatus is provided, resilient surfacing shall be in compliance with the American Society for Testing and Materials standard F 1292-99 as shown in the Compressed Loose Fill Synthetic Materials Depth chart and with Use Zones for Equipment.of the National Program for Playground Safety's Selecting Playground Surface Materials.

The resilient surfacing required is at least six inches underneath and surrounding the equipment.
1. On the Pre-K/School age playground, there was 1/4 inch of mulch 10 inches in front of the double slides, the partially enclosed slide and the climbing rungs to access the composite play structure.

2. On the Infant /Toddler/Two year old playground there was 1 inch of mulch, 10 inches in front of the red slide.

3. Once mulch degrades and turns to soil, it is no longer a resilient surface.
Plan of Correction: Mulch was added to the playground in March while the previous Director was overseeing school operations.
Since then, the landscaping vendor has returned to the school and applied additional mulch to ensure compliance with resilient surfacing requirements.

During the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency.

There is one staff with the school aged children in the second building after school. There is up to 16 children in the building with the one staff.
Plan of Correction: During the transition between the previous Director and the newly appointed Director, the Head of Schools became aware that this had been a standard practice.
On her first day, the new Director revised the staffing schedule to ensure that two staff members are consistently assigned to the school-age building at all times.
