Each immunization report shall contain the dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
The immunization record for child A did not contain a signature by a physician/designee/health department. This was overdue by 10 weeks.

Plan of Correction: We will inform families that they must request a signature from their doctor and we cannot accept just the doctors stamp on these forms.

Systemic Deficiency
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
1. Child A and B have an intolerance to cows milk and a dietary restriction to pork, none of which were listed on the current written list in the classroom dated 12/1/25. Failure to list these restriction/sensitivity were overdue by 14 weeks.
2. Child C has a lactose intolerance however the allergy/sensitivity/diet restriction list in the classroom (dated 12/1/25) did not list this intolerance. This was overdue by 6 months.
3. Child D has an allergy to eggs, wheat and acidic fruits. The 10/21/25 allergy/sensitivity/dietary restriction list in the classroom did not list any of these allergies. Failure to list these allergies were overdue by 10 weeks.
4. Administrative staff reported that the classrooms with a 12/1/25 allergy/sensitivity/dietary restriction list was not a current list as the current list is dated 10/21/25.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
The attendance board in the two year old classroom had 6 children marked as present, however there were 7 children present.
The attendance board in the preschool ( 3 to 5 years) old classroom had 12 children as present, however there were 10 children present.
