Standard 8VAC20-780-270-A
Repeat Violation:
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
The electrical room in the Pre-K Classroom was unlocked and when opened, electrical wires and electrical panel were accessible.
Plan of Correction: We will lock the door to the electrical room.

Systemic Violation:
Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom provided that the staff check on a child who has not returned from the restroom after five minutes and there is a system to assure that individuals who are not staff or persons allowed to pick up a child in care do not enter the restroom area while in use by children.

1. Child A, 5 years of age left their Pre-K classroom, shut the door behind them and entered the vacant multipurpose classroom. Child A was out of sight and sound supervision once they entered the multipurpose room.
2. Child A then proceeded to enter the bathroom in the multipurpose room, where they remained for approximately 4 minutes. During those 4 minutes there was no staff able to hear the child in the restroom.
Plan of Correction: I will make sure all staff know that when a child is ready to go to the bathroom they should first notify the front office who will escort the child to the bathroom.

Furnishings, equipment, and materials shall be of an appropriate size for the child using it.

1. The school age children are using the multipurpose room in the main building. The multipurpose room had 3 small tables and approximately 10 small chairs in the room. The tables and chairs were the appropriate size for preschoolers and not school aged children.

2. Eighteen school aged children were using this room on 2/24/26.
Plan of Correction: We will bring the furniture from the schoolage building and place it in the multipurpose room.

Materials and equipment available shall be age and stage appropriate for the children and shall include an adequate supply as appropriate for each age group of arts and crafts materials, texture materials, construction materials, music and sound materials, books, social living equipment, and manipulative equipment.

On 2/24/26, there were 18 schoolagers present in the multi purpose classroom. The multipurpose room contained an empty book shelf and 3 small tables and chairs meant for preschoolers.
Plan of Correction: We will move the supplies for the schoolage children into the multipurpose room.

Documentation shall be maintained of emergency evacuation drills.

The January 8, 2026 escape drill documented was missing the time of the drill, the method used for notification of the drill, the number of children participating, if there were any special conditions simulated, the time it took to complete the drill and the weather conditions.
Plan of Correction: I will make sure to document all of the components of an escape drill.
