During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on July 17, 2025, as a part of the conditional licensure period. The on-site inspection began at 8:15am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 49 children present and 13 staff. The inspector reviewed 10 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-330-F
A shady area shall be provided on playgrounds during the months of June, July, and August. The outdoor playground did not have any shady area for the children.
Plan of Correction: The provider responded with the following: A temporary large beach umbrella will be used until a permanent cover or awning will be installed.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the work phone number and place of employment of each parent who has custody. Six of the ten children?s files reviewed did not have a parent?s work phone number.
Plan of Correction: The provider responded with the following: The provider will ensure that all the work phone numbers are obtained today at pick up and will be placed in the child?s record. The center will check that all parents provide a work number.
