Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 7/9/25. The on-site inspection began at 9:25 am and ended at 10:20 am. The inspector reviewed compliance in the areas listed above. There was one child present and one caregiver; the point total was 4. The inspector reviewed one child's record on site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
8The license and the most recent findings were not posted in a conspicuous place on the licensed premises.
There was no physical on file for child A, the physical was overdue by one month.
The immunization record for child A was not signed by a physician/designee or health department official. This signature was overdue by 2 months.
There was no written record of children in attendance; there was one child present during the inspection.
The provider did not wash their hands before helping child A with feeding.
The diapering pad was not cleaned or sanitized after use.
Child A enrolled two months prior and this information was not provided to parents in writing.
The provider has had at least one child enrolled for two months and there have been no emergency evacuation drills practiced monthly.
