Inspector Notes
An announced on-site initial inspection was initiated and completed on 1/14/25. The on-site inspection began at 1:30pm and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There are no children enrolled at this time. Four caregiver/household member(s) records were previously reviewed as part of a complete application. This inspection included document review, tour of the facility and an interview.
Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the program.
