An unannounced, on-site monitoring inspection was completed on 5/5/2025, as a part of the Conditional licensure period. The on-site inspection began at 9:10 am and ended at 9:30 am. The inspector reviewed compliance in the areas listed above. There were no children enrolled. The caregiver and household members records are current.
This inspection included: document review, tour of the facility, and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
