Inspector Notes

An announced, on-site initial inspection was initiated on 2/10/2025 and completed on 2/10/2025. The on-site inspection began at 11:44 am and ended at 1:55 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 6 staff records on-site. This inspection included document review, a tour of the facility, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/18/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statement for Staff #2, Staff #4, Staff #5, and Staff #6 who are currently employed, was dated five days after their first day of employment.
Plan of Correction: Moving forward Director will ensure all staff sign sworn statements before date of employment. Staff members did not have contact with any children as we are not open for care yet.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #5, who had been employed for five days, did not have a completed national criminal background check.
Plan of Correction: Moving forward Director will ensure all staff have a criminal background before their first day of employment. Staff #5 will resume job once the background is received. Staff #5 did not have contact with any children as we are not open for care yet.
