Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 7/09/2025, as a part of the provisional licensure period. The on-site inspection began at 12:52pm and ended at 1:25pm. The inspector reviewed compliance in the areas listed above. There was one child in care and one resident child.present and one caregiver; the point total was 4. The inspector reviewed one child?s record and one caregiver/household member records on-site on 7/09/2025.
This inspection included document review, tour of the home, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3The findings from the most recent inspection, dated 2/06/2025, were not posted in a conspicuous place on the premises. The summary was inside the closet.
A deodorizer spray was in the bathroom behind the toilet that children use .
The record for child #1 was missing the following components: One parent's home address, one parent's place of employment and work phone number, a 2nd emergency contact name, address and phone number and the medical insurance policy number. The child has been in care two days.
