Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 12/09/2024. The inspection started at 1:30 pm and concluded at 2:30 pm. There was (1) child present and (1) caregiver, the point total was (4). The inspector reviewed (1) child?s record and (3) caregiver/household members records on-site on 12/09/2024.

Information gathered during the inspection determined a non-compliance with an applicable standard. This violation was documented on the violation notice issued to the home.

Before attendance, documentation of immunizations is to be obtained for each child.

There was no documentation of immunizations for Child #1, who was in care during the inspection and had been in care for over a month.
Plan of Correction: Parent will be reminded and immunizations shall be obtained and placed on file.
