Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 11/12/2025. The inspection started at 10 am and concluded at 11 am. There was (1) child present and (1) caregiver; the point total was (4). The inspector reviewed (2) children?s records and (3) adult caregiver/household member records on-site on 11/12/2025.
