During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 4/9/2025, as a part of the conditional licensure period. The on-site inspection began at 10:05am and ended at 1:05pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 7 staff. The inspector reviewed 7 children?s records and 4 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/16/2025. A POC submitted after this date will not appear on the public website.
Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
The sworn statement for Staff #1, who is currently working, was dated the day after their first day of employment.
Plan of Correction: Moving forward, all potential employees will be required to sign a sworn statement during the initial interview process and prior to their first day of employment.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.
National criminal background check for Staff #1 was completed 2 weeks after date of hire. National criminal background check for Staff #2, was completed 7 days after date of hire.
Plan of Correction: Moving forward a national criminal background check will be conducted and completed prior to the commencement of employment for all new hires.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #7, who had been attending for six weeks, did not have documentation of an immunization record on file.
Child #2, who has been attending for 16 days, did not have documentation of up to date immunization record on file DOL 9/2023.
Plan of Correction: Current immunization records have been received for child#7 and chld #2. Moving forward, both DIrector and Assistant Director will be responsible for verifying tht all immunization documentation is up to date.
Standard 8VAC20-780-530-A-1
At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. The training shall include an in-person competency demonstration.
Staff #1 did not have current CPR certification from an organization that included an in-person competency demonstration.
Plan of Correction: Staff #1 won't be in any area with children, unless there's another staff member present who is certified in CPR and First Aid. This protocol will stay in place until Staff #1 completes an in-person CPR and First Aid course.
Standard 8VAC20-780-530-A-2
At least one staff in each classroom or area where children are present shall have current certification in first aid from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council.
Staff #1 did not have current first aid certification.
Plan of Correction: Staff #1 won't be in any area with children, unless there's another staff member present who is certified in CPR and First Aid. This protocol will stay in place until Staff #1 completes an in-person CPR and First Aid course.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
The record for Child #5 did not have a signed agreement by the parent that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.
Plan of Correction: The parent of child #5 signed the agreement on April 9, 2025., acknowledging their responsibility to notify the center within 24 hours or by the next business day if their child or any member of the immediate household is diagnosed with a reportable communicable disease.
