Inspector Notes

An unannounced monitoring inspection was initiated on 9/30/2025 and completed on 10/6/2025 in response to a self-report received by the licensing office on 9/22/2025 relating to care and protection. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/13/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.

1. On 09/22/2025 at approximately 2:00pm pm, staff #2 fed an 8-month-old child, child #1, another child's bottle containing formula.
a. Administrative staff reported to the inspector, on 09/22/2025, that staff #2 had mistakenly given child #2's bottle to child #1.
2. Staff #2 stated that they fed child #2's bottle before realizing that they had fed child #1 the wrong bottle.
3. Administrative staff and staff #2 stated that both bottles were dated and labeled with each child's full name and that staff #2 failed to follow center protocol for checking bottles before feeding.
Plan of Correction: Bottles have been consistently labeled correctly with the date, child's name and contents in accordance with center policies and licensing regulations. The name to face procedure is actively in place and consistently followed to ensure accurate child identification.
The teacher has been retrained on all feeding procedures to ensure full adherence to safety guidelines and program standards.
Ongoing monitoring and periodic retraining will be conducted to ensure continued compliance and high quality care.
