During the inspection, the inspector reviewed the areas. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 2/6/2026 as a part of the licensure period. The on-site inspection began at 9:17am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 31 children present and 9 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
2Staff #1, who had been employed for four months, did not have a central registry search requested. Staff #1 was working with children.
The center did not submit request for search of a sex offender registry check from a state lived in the past five years for Staff #1. Staff #1 has been employed for more than four months and was working with children at the time of the inspection.
The center did not submit request for search of a sex offender registry check from a state lived in the past five years for Staff #4, Staff #4 has been employed for more than two months and was working alone with children at the time of the inspection.
The center's administrative staff will ensure that all required background clearances are conducted in a timely manner.
