During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site inspection was conducted on 4/11/2025 from 10:20am - 11:20am. There were 5 children and 2 caregivers present at the time of the inspection; totaling 12 points. The inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-50-E
Records and reports on children shall be maintained and made accessible to the departments representative for two years from the date of termination of services.

A record of attendance was only available for the current week. The provider stated they are thrown away at the end of each week.
Plan of Correction: I will make sure to keep attendance records.
Standard 8VAC20-800-90-A
Before a child may attend, the provider shall obtain documentation that the child has been adequately immunized.

The immunization record for Child #1 was dated two days after the first day of attendance.
Plan of Correction: I will make sure immunizations are obtained in time for any new children.
