During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on August 26 2025 and completed on August 26 2025, as a part of the conditional licensure period. The on-site inspection began at 9:30am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 17. The inspector reviewed 2 children?s records and 1 caregiver/household member records.

This inspection included a document review, tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on September 5 2025. A POC submitted after this date will not appear on the public website

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

The provider has not obtained a physical examination for Child #2. The provider is 3 weeks out of compliance.
Plan of Correction: After my visit I promptly I asked for the proper and additional documents needed. I now have a complete file.
Standard 8VAC20-800-60-B
A child's record shall contain the following information: Emergency contact information including: Name and Policy number of the child's medical insurance, first date of attendance, written authorizations of emergency medical care.

The provider did not ensure that Child #2, 2 years 11 months of age, had the required documentation on file. The record did not include required insurance information, the child's fire date of attendance, or written authorizations for emergency medical care.

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

The provider did not obtain immunization records for Child #2, age 2 years 11 months. The provider is 8 weeks out of compliance.
