Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/02/2025, as part of the conditional licensure period. The on-site inspection began at 9:00 a.m. and ended at 10:30 a.m. The inspector reviewed compliance in the areas listed above. There were 26 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Staff 1 completed orientation training 4 days late, Staff 2 completed orientation training 16 days late, Staff 3 completed orientation training 16 days late, and Staff 4 completed orientation training 9 days late.
Plan of Correction: Provider will ensure orientation is done within the required timeframe.

Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.

In the Infant Safari room, a child was observed being returned to group care after a diaper change without their hands being washed.
Plan of Correction: Staff will ensure children wash hands after diaper changes.
