Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

1. Staff #2, who had been employed for six days, had a completed national criminal background check dated five days after their start date.
2. Staff #3, who had been employed for three months, had a completed national criminal background check dated five days after their start date.

The center must obtain the results of the central registry check prior to the employee working alone with children.

Staff #2, who had been employed for approximately one week, did not have their central registry search results in the file. Staff #2 stated they had been working alone with children.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of Child #4, who had been enrolled for more than two months.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

Staff #4 is currently working and has a TB screening that was dated 8 days after their first day of employment.

Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

1. Staff #1 and Staff #4 completed orientation more than 30 days after their hire dates.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: Date and time of injury, name of injured child, type and circumstance of the injury, staff present and treatment, date and time when parents were notified, any future action to prevent recurrence of the injury, staff and parent signatures or two staff signatures, and documentation on how parent was notified.

The injury report dated 8/6/2025 was missing the following information: time of incident, staff present, future action to prevent recurrence of the injury, and staff/parent signatures.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

1. Two out of five of the refrigerated infant bottles were not dated.
2. One of the five bottles was not labeled.

For staff hired after March 1, 1996, documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.

1. Staff #3, who has been employed for three months, did not have completed references in their file.
2. Staff #4, who has been employed for more than four months, did not have completed references in their file. References were missing the following information : date of contact, name of person contacted, and signature of the person making the call.
