Inspector Notes
An announced, initial inspection was initiated and completed on February 14, 2025. The inspection began at 1;06 p.m. and ended at 2:50 p.m. The inspector reviewed compliance in the areas listed above. There were 1children present and 1 caregivers; the point total was 2. The inspector reviewed 1 child?s record, and 1 caregiver record, and five household members records.
This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, observations (this would include observations of photos, recordings, etc.), and
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/28/2025. A POC submitted after this date will not appear on the public website.

4Child #1's record was missing the child's medical insurance policy number.
The parents of child #1 were not provided in writing the following required information: policies for the administration of medications, whether or not there is liability insurance in force on the family day home operation, and provisions of the family day home's emergency preparedness and response plan. The child has been in care for three weeks.
There was no parent acknowledgement available for one child, who has been enrolled in the program for three weeks.
There was no written emergency preparedness plan available for review during the inspection.
