An announced, on-site initial monitoring inspection was initiated and completed on February 10, 2025. The on-site inspection began at 9:15am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were four children present and two caregivers; the point total was 10. The inspector reviewed four children?s records and four caregiver/household member(s) records on-site..
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The family day home shall have a written emergency preparedness and response plan.

The family day home did not have a written emergency preparedness and response plan completed.
Plan of Correction: The provider will complete the emergency preparedness and response plan today and send it to the licensing inspector. The provider will ensure that it includes the emergency evacuation, emergency relocation, and shelter in place procedures. The plan will also address the most likely to occur scenarios. The provider will use the department's model form to complete the plan.
