Inspector Notes

An announced monitoring inspection was completed on 06/16/2025 as a part of the licensure period. The on-site inspection began at 10:00am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility. There were 6 children present and 1 caregiver. The inspector reviewed 4 children?s records and 2 caregiver/household member(s) records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition and free of unsafe conditions such as entrapment hazards. The stairway leading up to the entrance of the licensee's home had spaces which posed a potential head entrapment hazard along the stair case.
Plan of Correction: A barrier will be placed in any area of the stair case that measures more than 3.5inches and less than 9 inches.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Child #1 and #3 who were in care at the time of inspection, did not have documentation that they had been adequately immunized.
Plan of Correction: Licensee will contact the parents to obtain immunization records that include the physicians signature and has indicated whether the child has been "adequately immunized."
