Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 19, 2026 and completed on March 26, 2026. The on-site inspection began at 11:42 am and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 19. The inspector reviewed 3 children?s records and 1 caregiver/household member(s) records on-site on March 19, 2026.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 2, 2026. A POC submitted after this date will not appear on the public website.

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #1 who had been in care for over 60 days.

Children's records are required to contain parental home and employment addresses and phone numbers. The record for Child #1 was missing name, address, and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The child was present during the was present during the inspection and had been in care for a month.
