Inspector Notes
An announced initial subsidy health and safety inspection (SHSI) was initiated and completed on 02/05/2025. The inspection started at 1:15pm and concluded at 2pm. The inspector reviewed 1 caregiver record and 3 household member records on-site on 2/5/2025 and electronically on 1/30/2025. This inspection included document review, tour of the home, and interview. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
