Inspector Notes
An unannounced, on-site focused inspection was initiated on and completed on 9/26/2025. The on-site inspection began at 12:50pm and ended at 2:oopm. The inspector reviewed compliance in the areas listed above. There were 32 children present and eight staff. The inspector reviewed two children?s records and six staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
The center has not requested a search of the child abuse and neglect registry or the sex offender registry from a state that Staff #3 resided in the past five years.
