Inspector Notes

An unannounced, on-site monitoring inspection was completed on 5/15/2025, as a part of the Conditional licensure period. The on-site inspection began at 9:20 am and ended at 11:40 am. The inspector reviewed compliance in the areas listed above. There were (85) children present and (29) staff. The inspector reviewed (8) children?s records and (8) staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and when change of ownership, staff TB results must not be older than 30 days or an Allowable Variance must be requested.
Staff #2, #4, #5, #6 and #7 had current TB results but were dated earlier than 30 days before the 3/1/2025 initial license was issued. An Allowable Variance was not requested.
Plan of Correction: An Allowable Variance for Staff TB results was requested, signed and submitted to the Inspector during this inspection.
