During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was completed on 8/4/2025, as a part of the licensure period. The on-site inspection began at 9:30 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were (76) children present and (22) staff. The inspector reviewed (7) children?s records and (7) staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-E
The center did not follow the procedure for administering medication, including the written authorization from the parents to administer long term prescription.
The medication authorization form dated 7/29/2025 for Child # 2 does not have written parental authorization.
Plan of Correction: The parent will sign the medication authorization form.
Standard 8VAC20-780-510-I
Prescription medication may be administered to the child identified on the prescription label in accordance with the prescriber's instruction pertaining to dosage.
Child # 3's medication authorization form dated 3/21/2025 says to administer 0.1 mg of medication. The prescription label says to administer .15 mg of medication.
Plan of Correction: The parent will obtain the proper medication and or authorization form for the child's medication. Staff will ensure there are no discrepancies between the medication, label and form.
