8VAC20-780 Staffing and Supervision

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on 10/2/2025 and completed on 10/2/2025 in response to a complaint received by the licensing office on 9/29/2025 relating to allegations of Staffing and Supervision. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The required ratio for children ages birth to 16 months is one staff per four children.
Staff #1 and #2 in the infant room were observed supervising ten infants at 8:45 am. After reviewing the sign in sheets, staff #1 was out of ratio from 8:07 am until 8:15 am. Staff #1 and #2 were out of ratio from 8:41 am until 8:48 am when another staff came to assist.

Staff #3 was observed at 8:45 in the toddler 1 room supervising 5 children, the youngest 13 months old. Staff #3 was out of ration from 8:33 am until 8:48 am when another staff came to assist.
Plan of Correction: The floaters will be moved into the infant room at 8:30 am and the other floater will be moved to the toddler one room at 8:45 am.
