Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 06/03/2025, as part of the conditional license period.
The on-site inspection began at 12:15 pm and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above.
There were 7 children present and 2 caregivers. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site.
This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 06/13/2025. A POC submitted after this date will not appear on the public website.
Violations
5The record for Caregiver #2, who has been caring for children for five weeks, did not contain documentation of having completed orientation training.
The record for Child 3, who has been in care for two days, was missing one parent's work phone number and the address for one emergency contact
The record for Child 4, who has been in care for two months, was missing one parent's work phone number and the medical insurance number.
Child 4 - work phone number added to existing contact numbers provided by parents. Medical insurance number not extant for that child.
The record for Child #2, who has been in care for two months, did not contain signed acknowledgement of having received the Provisions of the emergency plan of the fdh.
The emergency drills log did not have an evacuation drill documented for the month of May 2025.
