Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 06/03/2025, as part of the conditional license period.
The on-site inspection began at 12:15 pm and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above.

There were 7 children present and 2 caregivers. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 06/13/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-200-A
The provider shall orient the assistant by the end of their first week of assuming job responsibilities.

The record for Caregiver #2, who has been caring for children for five weeks, did not contain documentation of having completed orientation training.
Plan of Correction: Documentation of training on all requisite points confirmed and signed by assistant.
Standard 8VAC20-800-60-B
An up to date record for each child enrolled that contains all required information shall be maintained by the provider.

The record for Child 3, who has been in care for two days, was missing one parent's work phone number and the address for one emergency contact
The record for Child 4, who has been in care for two months, was missing one parent's work phone number and the medical insurance number.
Plan of Correction: Child 3 - work phone number added to already provided contact numbers for parent. emergency contact address added.
Child 4 - work phone number added to existing contact numbers provided by parents. Medical insurance number not extant for that child.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided in writing required information.

The record for Child #2, who has been in care for two months, did not contain signed acknowledgement of having received the Provisions of the emergency plan of the fdh.

The emergency evacuation procedures shall be practiced monthly.

The emergency drills log did not have an evacuation drill documented for the month of May 2025.
Plan of Correction: Emergency evacuation drill performed and recorded June 4. An additional drill will be performed in June to reinforce the practice.

The menu for the current one-week period shall be dated.

The menu posted was not dated.
Plan of Correction: Menus updated with dates for the one-week period to be used.
