During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated on 09/25/2025 and completed on 09/25/2025, as a part of the conditional licensure period. The on-site inspection began at 12:08pm and ended at 2:38pm. The inspector reviewed compliance in the areas listed above. There was one child present and one caregiver; the point total was 2.
The inspector reviewed two children?s records and one caregiver record and one household member record on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3A child?s record shall contain all required information.
Child #1, enrolled for one month, was missing all of the emergency contact information, parent?s signed acknowledgement of written information for parents, written authorization for emergency medical care.
Child #2, enrolled for one month, was missing all of the emergency contact information, parent?s signed acknowledgement of written information for parents, written authorization for emergency medical care, and written permission for transportation and field trips.
The caregiver was missing the August 2025 evacuation drill.
The provider?s transportation vehicle did not contain emergency supplies.
