During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 9/23/2025 and completed on 9/29/2025. The on-site inspection began at 11:37 am and ended at 1:10 pm. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 4. The inspector reviewed 3 children?s records and 4 caregiver/household member(s) records on-site on 9/23/2025.

This inspection included document review, tour of the facility, interviews, observations, and measurements
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-770-40-D-5
A person living in the licensed home 14 years of age and older must have a search of the central registry and make the information available for regulatory purposes within 30 days of becoming 14 years old.
Household member #4 did not have results of a search of the central registry which was nearly 4 months overdue.
Plan of Correction: The Central Registry request for household member #4 was submitted during the inspection and the results of the Central Registry Search were received and forwarded to the inspector on 9/24/2025.
Standard 8VAC20-800-460-D
The provider shall maintain documentation of the current rabies vaccination for pets residing in the home. One pet (#1was overdue for an updated rabies vaccination by 3 days.
Plan of Correction: Household Pet #1has an appointment for an updated vaccination on 9/26/2025.
Standard 8VAC20-800-60-B
Enrolled children's records must contain the required information. Child #3 had minimal information in their record and a complete file was overdue by one day.
Plan of Correction: The required paperwork and documentation will be provided, again, to the parent of child #3 today and required to be completed and turned in to the provider today. Families will be required to submit their enrollment paperwork in full prior to children being cared for.

The family day home must have a First Aid kit which contains all of the required contents.
The First aid kits available in the family day home did not contain triangular bandages.
Plan of Correction: An order was placed for triangular bandages during he inspection and documentation showing they were received was sent to the inspector on 9/25/2025.
