During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 2, 2025, as a part of the conditional licensure period. The on-site inspection began at 10:10 am and ended at 11:23 am. The inspector reviewed compliance in the areas listed above. There were 2 children present, 1 household child, and 2 caregivers; the point total was 9. The inspector reviewed 2 children?s records and 1 caregiver, and 2 household member records on site. This inspection included document review; interviews, and observations Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 16, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #2, who had been in care for over 40 days.

Standard 8VAC20-800-60-A
Children's records are required to obtain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Child#2 record did not include two emergency contacts. The child was present during the inspection and had been in care for over 1 month.
