Inspector Notes

An unannounced, on-site initial inspection was initiated and completed on 5/8/2025. The on-site inspection began at 9:30am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were no children enrolled. The inspector reviewed 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The ceiling vent cover in classroom #2 was not secure. The screw was pulling out and the corner of the vent cover was pulling away from the ceiling. The rain gutter on the playground was broken and had jagged edges.
Plan of Correction: The ceiling vent and the gutter have been repaired.
