Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 6/4/2025 and completed on 6/6/2025. The inspection started at 10:35 am and concluded at 12:15 pm. There were no "non-household" children present and 1 caregiver; the point total was 5. The inspector reviewed 2 caregiver/household members records on-site.
This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program
Standard 22.1-289.036-B-4
The vendor is required to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. The vendor did not obtain a search of the sex offender registry or Criminal Record Search from one state for staff / household member #1 and #2 who are currently working with children and / or residing in the home where children are cared for and for whom a subsidy vendor application has been submitted.
Plan of Correction: The out of state sex offender registry and the out of state Criminal Record Search from one state for staff / household member #1 and #2 were obtained on 6/4/2025 and forwarded to the inspector.
Standard 8VAC20-790-360-G
Cots, rest mats, or beds must be provided for children 12 months of age or older. The vendor did not have cots, rest mats, or beds accessible to the 2 children she cares for.
Plan of Correction: Rest mats were purchased for children's use following the inspection and order documentation was sent to the inspector 6/4/2025.
Standard 8VAC20-790-410-B-2
The vendor is required to have a battery-operated radio as part of her emergency supplies. The vendor stated that she did not have a batter-operated radio in the home.
Plan of Correction: A battery operated weather band emergency radio was purchased following the inspection and order documentation was sent to the inspector 6/4/2025.
Standard 8VAC20-790-420-A
The vendor is required to have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures.
The vendor's written emergency plan did not include lockdown procedures.
Plan of Correction: The Emergency Plan was updated to include Lock Down Procedures.
