During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 05/07/2025. The on-site inspection began at 3:30 p.m. and ended at 5:00 p.m. The inspector reviewed compliance in the areas listed above. There were five children present with two staff. The inspector reviewed three children?s records and three staff records on-site.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision. Staff members must have a complete annual "Staff Health Report" on file.

Staff A who has been employed for two days did not have a staff health report on file.
Plan of Correction: Management will ensure that for all future inspections all staff documents are accessible at the time of the inspection.

The program is required to ensure that staff are able to recognize the signs of child abuse and neglect.

The two staff on site did not have evidence of completing training to recognize the signs of child abuse and neglect.
Plan of Correction: All staff have completed all missing trainings and are in compliance.
