The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. S3, employed for more than 2 months, did not have documentation of requesting a central registry check or criminal history check from one state.

Staff shall complete orientation training prior to the staff member working alone with children and no later than 7 days of the date of assuming job responsibilities.
S2 and S3 completed training 9 days of the date of assuming job responsibilities.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, licensing inspector observed 8 children, age 3, unsupervised in their classroom for approximately 2 minutes before the staff entered the classroom.

In addition to staff required by rest period ratios, an additional staff member shall be always available on-site to offer immediate assistance. Upon arrival to the center, the licensing inspector observed that all staff members were in coverage with children with no additional staff onsite.

When food is brought from home the food container must be clearly dated and labeled in a way that identifies the owner. A plastic container of food and food pouch brought from home were not labeled with a name or date in the infant room.
