During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 3/19/2026 and completed on 3/23/2026 in response to a self-report received by the licensing office on 3/13/2026 relating to supervision. The inspector reviewed 1 child?s record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/1/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #1, enrolled for over 3 months, did not have documentation of an immunization record on file.
Plan of Correction: Immunization records have been requested from the parent.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #1, who had been enrolled for over 3 months.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On March 13, 2026, staff were unaware that child #1 (age 4) climbed over the playground fence and entered the roadway which has a posted speed limit of 30 miles per hour. Witness #1 reported seeing the child walking into the road from the second story of the building next to the center. Witness #1 immediately ran from the building, down the stairs, out an emergency exit and into the roadway to rescue Child #1 from oncoming traffic.

Standard 8VAC20-780-340-F
Children under 10 years of age must always be within actual sight and sound supervision of staff.

On March 13, 2026, child #1 (age 4) was reunited with center staff after witness #1 rescued the child from oncoming traffic on a nearby road. Child #1 was out of sight and sound supervision for approximately 2 minutes and 20 seconds.
Plan of Correction: Staff have been retrained on proper supervision.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by the standards.

1. The center's policy for playground supervision states: ?Active supervision shall be maintained at all times on the playground. Teachers shall maintain ?active sweeping? supervision of the playground at all times.?

On March 13, 2026, Child #1 (age 4) climbed over the playground fence, approximately 30 inches in height, and wandered into a nearby roadway located approximately 171 feet from the center?s playground. Staff #1 was seated on a bench supervising this child?s group, consisting of eight children, while staff #2 returned inside to retrieve an item. Staff #2 returned to the playground and sat at a table with two additional staff, who were supervising another group of children, when witness #1 returned the child to center staff. None of the staff were aware that child #1 had climbed over the fence and wandered into the road.

On March 13, 2026, child #1 was out of sight and sound supervision of center staff for approximately 2 minutes and 20 seconds. During which time, child #1 climbed the playground fence and wandered into a nearby roadway.
Plan of Correction: Staff have been retrained on FHCCC's playground safety plan, which outlines proper supervision policies and procedures on our playground. Staff have been assigned additional trainings on this topic.
