During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/04/2025, as a part of the conditional licensure period. The on-site inspection began at 9 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (8) children present and (1) caregiver; the point total was (16). The inspector reviewed (10) children?s records, and (2) caregiver records on-site on 06/04/2025.

This inspection included: document review (i.e. Files, injury logs (none), policies and procedures, emergency drill logs, medication authorization forms); tour of the facility; interviews; and observations.

Information gathered during the inspection determined (1) non-compliance with an applicable standard, and this violation was documented on the violation notice issued to the program.

The Provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There were no physical exam records available for Child #4, who had been in care for over 60 days.
Plan of Correction: Corrected on the date of inspection. The Provider emailed this documentation to the Licensing Inspector for review.
