The provider shall maintain up to date records for each child enrolled that contain all of the required information. There was only one emergency contact listed in the records of child #1 and child #2. Also, there was no physician contact information in the records of child #1 and child #2, who were in care for almost one month.
Plan of Correction: One of the missing emergency contacts has been updated in our online application system. All future changes to emergency contact information will be reviewed and signed by parents. The physician contact information has been fully updated in our online application system.
Standard 8VAC20-800-90-A
Before attendance, documentation of immunizations is to be obtained for each child. There was no documentation of immunizations for child #1 and child #2, who were in care for almost one month.
Plan of Correction: Enrollment checklist has been added to our Parent handbook to ensure all required health forms such as immunization records are received and verified upon enrollment. Enrollment services will not be finalized without required immunization records on file.
