Repeat Violation
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
The immunization forms for Child #1 and Child #3 were not signed and the physical examination form for Child #2 was not signed, which is required.
Plan of Correction: We are actively reviewing all children?s files to confirm that shot records are properly stamped or signed. Moving forward, all new children?s shot records will be verified prior to enrollment to ensure this requirement is consistently met.

Repeat Violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In a pre-toddler classroom, there were mop cleaning pads stored in an high, unlocked cabinet. In another pre-toddler classroom, there were disinfectant wipes, disinfectant sprays, and a bleach/water solution stored in a low, unlocked cabinet. In a toddler classroom, disinfectant spray and a bleach/water solution were stored in a high, unlocked cabinet.
Plan of Correction: All chemicals have been removed from lower cabinets and placed in secured upper cabinets. All upper cabinets now have functioning locks. Staff have been retrained and reminded to lock cabinets immediately after removing or returning any product. Management will sign the ASYMCA True-Up Form three times per day. Completed forms will be stored in the True-Up binder, organized by month.

The diapering area shall have the following: a sink with running warm water not to exceed 120?F.
In two infant classrooms, the water temperature exceeded 120?F.
Plan of Correction: Water temperatures have been adjusted through the facility app to ensure they remain below 120?F. Classroom staff will check and log water temperatures each morning and afternoon. Temperature adjustments can be made via the app located on the Executive Office computer and management cell phones.

At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care. The training shall include an in-person competency demonstration.
In an infant classroom and in a preschool classroom, there was not at least one staff member with CPR certification that met the in-person competency requirement.
Plan of Correction: All CPR and First Aid certifications have been reviewed. Staff who have not completed the in-person CPR component will be retrained by December 9th. Until retraining is complete, there is always at least one staff member fully certified in CPR/First Aid present with the children at all times.
