When staff are supervising children, they shall always ensure their care.
On 11/18/25, Staff #1 gave a bottle to Child #1 that was not the infant's bottle. The infant consumed approximately three ounces of the bottle of milk, which caused a reaction. The staff member did not confirm that the bottle was the correct bottle beforehand.

Plan of Correction: All staff will review and be retrained on the center?s face-to-name policy. Staff will ensure that the infant?s name is checked on the bottle when it is removed from the refrigerator, checked again when placed in the warmer, and verified a third time immediately prior to providing the bottle to the infant.

Staff will receive a copy of Safe Feeding Practices for Infants Under 12 Months of Age and will be retrained on accurately and consistently completing daily infant documentation in ProCare, ensuring all required information is entered each day and is accessible to parents and staff. This will be completed by 01/12/2026 with a sign-off form.
